  General OMSB Evaluation Tools (list of evaluation forms only)
The evaluation at the end of the rotation is made by the supervisor/consultant responsible for the resident during that rotation.  Supervisors are required to give continuous verbal feedback during the rotation.  The resident bears the responsibility for ensuring that the evaluations are completed in a timely fashion.  Residents who rotate in other departments/specialties are expected to perform and receive evaluations for the following activities, in addition to the In-Training Evaluation Report (ITER):
 
1.  Presentations (CMEs, Journal clubs, etc.)
2.  Procedural skills as listed in the logbook
 
Residents are also expected to evaluate the rotation and the trainer/s by the end of each block.
 
  Program-Specific Evaluation Tools 
Block assessment in FAMCO Clinic:

  •   Each resident will be assigned to one team.
  •   Block Assessment will be done by the consultant of the team. The residents are advised to introduce themselves to their respective consultants in the beginning of the block and arrange in advance a convenient time to do the block evaluation.
 
  •   Resident posted in the FAMCO Clinic is required to complete the following evaluations:
1.  Complete Record of all Patients Seen
2.  ONE Medical Record Note (selected randomly)
3.  Any ONE of the following: (should be different per each block)
- Mini-CEX
- Case-Based Discussion (CBD)
- Professionalism Mini Evaluation Exercise
- Observed Long Case (OLC)
4.  ONE Linking Learning to Practice (LLP)
5.  ONE Critical Appraisal (CA)
6.  ONE Teaching Activity (Residents as Teachers)
7.  ONE Managerial activity (Resident as Manager)
8.  FIVE Filled Evening & Weekend Shifts Evaluation Forms
9.  ONE Professionalism Evaluation Form - to be done once every 4th block
10.  Update on Research Project
 
  Block assessment in the Local Health Center (LHC):
  •   The resident is responsible for getting the block evaluation form filled in by the trainer at the end of each block.
  •   The resident should arrange in advance a convenient time to carry out the evaluation, and the evaluation should be discussed with the resident.
  •   Resident posted in the LHC is required to complete the following evaluations:
1.  Complete Record of all Patients Seen
2.  ONE Medical Record Note (selected randomly)
3.  Any TWO of the following: (should be different per each block)
- Mini-CEX
- Case-Based Discussion (CBD)
- Professionalism Mini-Evaluation Exercise
4.  TWO Linking Learning to Practice (LLP)
5.  ONE Professionalism Evaluation Form
6.  FIVE Filled Evening & Weekend Shifts Evaluation Forms
 
  Clinical Day Release assessment in the Local Health Center (LHC):
  •   The resident is responsible for getting the clinical day release evaluation form filled in by the trainer at the end of each block.
  •   The resident should arrange in advance a convenient time to carry out the evaluation, and the evaluation should be discussed with the resident.
  •   Resident is required to complete the following evaluations:
1.  Complete Record of all Patients Seen
2.  ONE Medical Record Note (selected randomly)
3.  Any ONE of the following: (should be different per each block)
- Mini-CEX
- Case-Based Discussion (CBD)
- Professionalism Mini-Evaluation Exercise
4.  ONE Linking Learning to Practice (LLP)

1.  Research Elective Checklist
2.  Research Block Evaluation Form
3.  Research Mentor Evaluation Form
